Medicare
Insurance payments are negotiated with insurance companies. FL Nutrition Group, LLC is currently contracted with Medicare. Under Medicare regulations, the following conditions will be covered for nutrition counseling:
Diabetes
Renal disease
36 months post kidney transplant
In addition to having one of the three medical conditions, Medicare also requires a referral from your physician for nutrition counseling. It is the patient’s responsibility to obtain this referral prior to your initial appointment, as we cannot bill Medicare without a referral.
Patients and/or doctors offices can send referrals to Florida Nutrition Group LLC via one of the following methods:
Ask your doctor’s office to fax the referral directly to FL Nutrition Group LLC at 754-778-5634
Scan a paper copy of the referral and email it to
FLNutritionGroup@gmail.com
